This acquisition of data report can be separated into two parts. One is health inspection related data which includes health inspection report, health questionnaire. The other is the tracking related data after the health screening, includes the result tracing, and nutritional counseling.
A: Health inspection related data
★Health inspection report
The health inspection data in four clinic computer centers will be sent back to the information center of administration of headquarter. All data will be integrated at the information center.
★Health questionnaire
The health questionnaire includes the data of personal general information, specific life habits (Ex. Smoke, alcoholic drink and chew betel nut), work status, dietary, personal and family diseases history, and recent health condition, etc..。
 The related data after the health screening
★Case tracing
According to the result of health inspection, if clinic doctor gives a diagnosis should have a case tracing to the subject, the senior nursing consultant will phone initiatively, keep on tracing, and record the result.
★Nutritional counseling
If the subject is a chronic disease patient, according to the subject’s health condition the nutritionist will give a nutritional counseling lesson when the subject receives the health inspection. Or, after health inspection the subject can make an appointment with a nutritionist for having a nutritional counseling.
